One of the key components of a successful patient discharge is to have an effective medication reconciliation process in place. Healthcare settings must understand the true importance of medication reconciliation and how and where to implement it within the discharge process. In this course—an installment in the Medication Reconciliation series—learn how to effectively educate and prepare your patients for a successful discharge. Along the way, find out what key elements need to be included when creating a thorough discharge medication plan, as well as key services and resources to arrange to assist the patient during and after discharge. By the end of this course, you’ll also be equipped with a richer understanding of the pharmacy and the role it plays in medication reconciliation throughout the discharge process.
