This course—the fourth installment in the larger Medication Reconciliation series—covers the key components of the medication reconciliation process during a hospital inpatient stay and steps to improve current med rec programs. Instructor Lesa Schlatman, RN, BSN, explains how to assess patient needs related to medication and build an education plan that addresses those needs while accommodating for patient preferences and barriers to learning. Lesa also identifies resources and tools to support the education process.
